How do you decide on a new career?

A question that has been nagging at me my whole life…I think it’s great that so many folks know exactly what they want and who they want to be.  I wish I was one of them!  I serendipitously ran into a colleague at work yesterday (she is a strong believer in the religion of serendipity and I think I’m beginning to be as well!), who is at a similar juncture in her career.  Have been with the company for almost 13 years, so have a lot of experience and history, but how does that translate into what to do with your future?  She recommended a book to me which I picked up at Barnes and Noble last night and can’t wait to get started on it, sounds very insightful and got great reviews on the BN and Amazon websites:
